(Adnkronos Salute) - "Alla prima perdita di urine, una spia che dovrebbe far riflettere sulla situazione anormale, è bene rivolgersi immediatamente all'urologo. Vale per gli uomini come per le donne, le più colpite dall'incontinenza urinaria. Il sintomo va approfondito e discusso con lo specialista che valuterà le cause che la determinano e il tipo di trattamento su misura per ciascun paziente". Così all'Adnkronos Salute Vincenzo Ficarra, professore ordinario di Urologia e direttore della Cattedra di Urologia presso l'Università degli Studi di Messina, in occasione della Giornata nazionale dell'urologia promossa dalla Società italiana di urologia (Siu) e che si celebra oggi. A scatenare l'incontinenza urinaria sono diversi motivi. "Nel caso si tratti di una perdita di urine che dipende da iperattività della vescica, questa iperattività può essere trattata con i farmaci - spiega Ficarra che è anche responsabile dell'Ufficio scientifico Siu - Abbiamo a disposizione gli anti-muscarinici o i Beta-3 agonisti, farmaci in grado di controllare la funzionalità della vescica". Un'altra causa dell'incontinenza urinaria "nelle donne - sottolinea lo specialista - può dipendere dal deficit dello sfintere, in genere accade dopo una gravidanza o è dovuto al prolasso degli organi uro-genitali. In tal caso procediamo con interventi chirurgici mininvasivi, così da consentire alle donne di ripristinare la loro continenza". Ci sono, inoltre, cause dovute alle fistole: "Si possono verificare comunicazioni anomale tra vescica e vagina che determinano l'incontinenza. Anche in questo caso procediamo con l'intervento chirurgico". Negli uomini, invece, la perdita di urina può essere "la conseguenza di interventi chirurgici per tumori o patologie benigne. Anche in questo caso la soluzione è l'intervento chirurgico mininvasivo, fino al posizionamento dello sfintere artificiale". Per evitare l'uso di pannoloni, che ha anche un costo economico per i pazienti, "abbiamo le terapie farmacologiche che però sono tutti a pagamento e vanno a pesare sulle tasche di 5 milioni di persone, tanti ufficialmente sono gli con incontinenza urinaria - conclude Ficarra - ma è un dato per difetto, sottostimato perché in molti per pudore non si rivolgono al medico, non ne parlano con l'esperto.
