(Adnkronos Salute) - Lasciare la corsa elettorale per il presidente Biden è stato complesso non solo a livello politico, "ma anche sul piano psicologico". Ha dovuto maturare una "consapevolezza, non facile, dei suoi limiti", e arrivare "all'accettazione della fine di una fase della sua vita" in modo "improvviso, drammatico" e contrario ai piani preparati da tempo. Questa in sintesi l'analisi all'Adnkronos Salute dello psichiatra Giancarlo Cerveri, consigliere della Società italiana di psichiatria (Sip), in merito alla lunga resistenza del presidente Usa all'abbandono della campagna elettorale per un nuovo mandato alla Casa Bianca. "La scelta di Biden sollecita, oltre a rilevantissime riflessioni politiche, anche una questione profondamente personale di ordine medico", osserva lo psichiatra. Già da diverso tempo, ricorda, "erano stati descritti momenti in cui Joe Biden era stato visto assente o distratto". Una situazione deflagrata nel "faccia a faccia con l'avversario Donald Trump". Una discussione e un dibattito, quelli sul presunto declino cognitivo, "particolarmente complessi per due motivi - spiega Cerveri - Il primo è che i sintomi cognitivi sembrava avessero un andamento fluttuante, in alcune occasioni erano evidenti e in altre non si osservavano. La giustificazione della prestazione di Biden è stata 'a bad night', come se il giorno dopo tutto ritornasse nella norma. Secondo elemento: per il meccanismo elettorale era di gran lunga preferibile che fosse lo stesso Joe Biden a rinunciare, dunque era lo stesso Biden che doveva sviluppare un livello di consapevolezza delle sue difficoltà tale da avere la forza di fare un passo indietro, con tutta la difficoltà, anche emotiva, di rinunciare a combattere quella che doveva essere la sua battaglia". Sul piano medico, "non avendo a disposizione esami e referti non abbiamo certezze - precisa lo psichiatra - ma si può ipotizzare che si sia di fronte a quella condizione che viene definita nel mondo medico Mild Cognitive Impairment (decadimento cognitivo lieve Mci), una condizione di declino cognitivo delle persone anziane non sufficientemente marcato per definire una vera malattia, ma abbastanza rilevante da ridurre il funzionamento quotidiano in modo nettamente superiore al fisiologico declino tipico dell'invecchiamento". E' una condizione che "tipicamente peggiora sotto stress cerebrale o sistemico: una notte insonne, un'influenza o altra infezione, l'introduzione di un nuovo farmaco, la disidratazione per il caldo e così via. Questo spiega il suo andamento fluttuante". Questa condizione "è particolarmente rilevante quando colpisce persone anziane che rivestono ancora ruoli attivi e importanti, perché chi ne soffre tende a difendere la sua posizione quando sta bene (e dunque con un funzionamento cognitivo integro) da persone che gli rinfacciano una condizione cognitiva fragile", spiega ancora lo psichiatra sottolineando che, per queste persone, c'è la difficolta di "accettare che in alcuni momenti il proprio cervello funziona in modo meno efficiente e che dunque deve abbandonare il ruolo che ha perché risulta poco affidabile per l'istituzione o azienda che dirige". In tutto questo "deve fare i conti con la fine di un’epoca della sua vita in modo improvviso e a volte drammatico". Spesso "sono individui che hanno avuto ruoli di grande rilievo anche in età avanzata e che in poco tempo sono costretti a fare i conti con la senescenza. Sono spesso individui che vanno incontro anche a reazioni depressive in taluni casi molto profonde", puntualizza Cerveri.La condizione decadimento cognitivo lieve, conclude, "è diffusa nella popolazione anziana. Si stima che circa il 15% del totale la viva.
