(Adnkronos Salute) - “Contrapporre ospedale e università, alimentando una contrapposizione ideologica tendente a stroncare la funzione per la quale dovrebbe esistere l’università e svilendo la funzione stessa dell’ospedale, è un errore madornale che vogliamo urlare a tutti. Un percorso formativo di qualità, per definizione, è sempre composto dalla giusta miscela di teoria/conoscenza e pratica/competenza. Altrimenti si arriva al disastro di oggi” per questo il sistema formativo ha bisogno di una riforma seria e concreta”. Così Antonello Giarratano, presidente Società italiana di anestesia analgesia rianimazione e terapia intensiva (Siaarti), in una lettera aperta al direttore di Quotidiano Sanità, Luciano Fassari, esprime la sua posizione volta a superare la contrapposizione ideologica tra ospedalieri e universitari, nel dibattito aperto, negli ultimi giorni, sul parere del ministero dell’Università e la ricerca (Mur) che intende ripristinare l’esame per il passaggio d'anno dei medici specializzandi assunti con il cosiddetto ‘Decreto Calabria’, che è stato accolto dall’opposizione di alcune associazioni di categoria dei medici specializzandi e di alcuni assessori regionali del welfare. La società scientifica Siaarti, nella lettera del presidente, oltre a ribadire che la sicurezza dei pazienti e la qualità della sanità devono essere al primo posto, evidenzia la necessità di una riforma urgente della formazione post laurea per gli specializzandi, nella quale non si possono prevedere contratti in ospedali periferici con una certificazione finale di competenze che, in quel contesto, non si potrebbero acquisire. “Il nostro sistema formativo - spiega Giarratano - ha bisogno di una riforma seria e concreta, lontana da condizionamenti ideologici. Solo attraverso un’adeguata formazione possiamo garantire ai futuri medici la competenza necessaria per affrontare le sfide della sanità moderna. È essenziale - chiarisce - mantenere standard elevati per garantire che i medici siano adeguatamente preparati. Nella parcellizzazione formativa attuale, uno specializzando di secondo o terzo anno dovrebbe andare, con un contratto a tempo determinato che lo vincola - per restare alla mia specialità di anestesia, rianimazione, terapia intensiva e del dolore - in un ospedale anche periferico a formarsi dove non ‘vedrebbe’ mai un neonato, un paziente neurochirurgico, un paziente cardiochirurgico o una procedura di terapia del dolore avanzata o di area intensiva e tanto tanto altro”. Sull’esame, che è “secondo un 44 quater scritto francamente in modo poco comprensibile”, il presidente Siaarti osserva che “consisterebbe in una certificazione delle procedure svolte che, in molti degli ospedali dove alcune regioni vorrebbero colmare i buchi, nessuno potrebbe certificare semplicemente perché non si fanno”. A questo, si aggiunge il fatto che “poi a fine anno con questa certificazione ‘farlocca’, un consiglio di docenti dovrebbe, facendo finta di non sapere che quelle attività non le ha svolte e senza alcuna valutazione anche degli studi e conoscenze acquisite, certificare che si è specialista? A fine anno faremo esami ai medici in formazione con 2 o 3 modalità diverse, per chi ha accettato di essere assunto e per chi invece è rimasto nel percorso ‘tradizionale’?”, domanda retoricamente Giarratano. Il presidente Siaarti, evidenziando le numerose criticità del sistema attuale, propone però una soluzione. “Facciamo una riforma, facciamola seria abbandonando il sistema per decreti e per articoli, che porterà il caos a fine anno alla prossima tornata di esami o di diplomi e anche come società scientifiche, non solo come docenti che tanto fanno per la formazione. Abbiamo idee e contributi che possiamo portare ai tavoli del legislatore puntando al risultato che a tutti interessa: la qualità dei nostri futuri medici”, conclude Giarratano.
